This study aimed to translate, culturally adapt and validate the Persian version of the Musculoskeletal Health Questionnaire (MSK-HQ) for measuring the health status of the musculoskeletal system. Methods: This cross-sectional study included forward and back translation the MSK-HQ following the International Society for Pharmacoeconomics and Outcomes Research guidelines, as well as preliminary testing, to achieve the final Persian version of the MSK-HQ (MSK-HQ-P). Cronbach’s alpha and the intraclass correlation coefficient were used to assess the internal consistency and test-retest reliability of the MSK-HQ-P on 152 patients with MSDs in Tehran, Iran, using an available and simple sampling approach. Spearman’s correlation was also utilized to evaluate the correlation between the MSK-HQ-P and the European quality of life five-dimension, five-level scale (EQ-5D-5L). Results: The mean MSK-HQ-P score was 37.81 ± 9.93. The instrument had a good level of internal consistency, Cronbach’s alpha (0.913), and reliability (ICC = 0.93). A strong association was also found between the MSK-HQ-P and the EQ-5D-5L scores (r = 0.761, p < 0.001). Conclusion: The Persian version of MSK-HQ was a valid and reliable tool for assessing MSK health in the Persian-speaking population with musculoskeletal disorders.
